   This practice is dedicated to maintaining the privacy of your protected health information (PHI). We are required by law to maintain the confidentiality of health information that identifies you and to provide you with this notice of our legal duties.
    

2. 2026 Substance Use Disorder (SUD) Protections
   In accordance with 42 CFR Part 2 and the 2026 HIPAA alignment rules, specific protections apply to records regarding substance use disorder. These records will not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without your specific written consent or a court order meeting the requirements of 42 CFR Part 2.
    

3. Your Rights Regarding Your PHI
   You have the right to request restrictions on certain uses, to receive confidential communications, to inspect and copy your PHI, and to request an accounting of disclosures.
